We are a unique South Asian Adult Medical Day Care Center in New Jersey. There are other facilities that provide services for South Asian Seniors along with other cultures within the same program. We are different! Although we are open to all cultures, we created an environment based on our rich South Asian culture and unique background. SHAKTI ADULT DAY CARE “Symbol of Strength”.
Hiral Patel, the owner of SHAKTI ADULT DAY CARE, has uniquely designed this 6,000 sq. ft. facility which reflects the unique and rich South Asian background which we come from.
With over seventeen years of experience in assisting clientele with the Hudson County Social Services and the Welfare Department, his main focus was having a unique hands-on experience in helping various age groups with problematic situations.
Throughout his life, he has been fortunate to have friends, co-workers, and seniors from the South Asian Community. He is comforted by the unique Asian cultural ways, much like the Indian tradition that he grew up with.
He currently advocates and is the Liaison for the Hudson County Social Services, representing the Hudson County under the Fair Hearing Department, which conducts hearings in front of a judge on a daily basis.
Furthermore, he sits as a certified licensed Administrator in the database of the New Jersey Department of Health.
Understanding the South Asian community through years of research, Hiral Patel has designed the most beautiful center for South Asian seniors.

Who do you service?
We offer assistance to individuals aged 18 and above who require supportive services to maintain independence at home, thereby averting institutionalization. Our Adult Day Care Centers are community-oriented programs tailored to address the needs of adults with functional or cognitive impairments, featuring personalized care plans. Through our structured and comprehensive programs, we deliver a range of medical, health, nutritional, social, and other supportive services in a safe and nurturing environment.
Do you get surveyed?
Indeed, once annually, surveyors assess our program, documentation, and service quality to ensure compliance. We consistently maintain good standing with the State Survey Process.

We hold licensure from the Department of Health and Senior Services as Adult Day Care Providers.
What types of services do you offer?
At SHAKTI DAYCARE, known as the “Symbol of Strength,” we provide a secure, nurturing, and enjoyable environment for your mornings and afternoons. Whether you prefer an active or relaxed pace, we offer a variety of engaging activities. Our compassionate and well-trained staff is committed to ensuring your well-being, enjoyment, and long-term health.
Social Service: Led by licensed Social Workers, we provide individual and family counseling, aid in accessing essential services like food stamps and Medicaid eligibility, as well as offer housing assistance and community service case management.
Medical Services: We have a licensed Medical Consultant and our Team oversees our program to ensure quality healthcare services. We coordinate with your primary physician to communicate your healthcare issues or concerns and to make sure that you maintain your health and well-being.
Nutrition: Guided by a registered dietitian, we serve two nutritious hot-catered Indian meals daily, along with snacks. Our services include dietary monitoring, counseling, and nutrition education. With a focus on South Asian culture, we offer a family-style Indian meal service.
Nursing: Supervised by our registered nurses, we deliver assessments, treatments, health education, medication management and administration, health monitoring, and comprehensive coordination of medical services. Additionally, we offer personal care, and emergency assistance including 911 calls as required, and conduct blood pressure and glucose testing and monitoring.
Therapeutic Activities: Led by our Activity Professionals, we offer physical, social, cognitive, and creative activities aimed at nurturing individual strengths and promoting well-being. Our daily activities cater to diverse interests, including exercise, games, parties, arts and crafts, movies, clubs, books, special events, educational programming, and entertainment. Additionally, we provide specialized activities tailored for Indian seniors and Person-Centered Care and Services.
Rehabilitation Therapies: Our licensed therapists, who are affiliated with us, offer physical therapy services and aid in obtaining specialized equipment. We facilitate referrals for Occupational Therapy, Speech, and Hearing Therapy to local providers, all coordinated by our medical staff.
Transportation: Our upscale vehicles offer door-to-door transportation services, accommodating assistive devices like walkers and wheelchairs. Additionally, we facilitate transportation to medical appointments.
Pharmacy: All prescriptions are filled and delivered daily. Medications are reviewed by our local registered pharmacist.
What Counties do we cover?
We serve Middlesex, Union, and Somerset, as well as portions of Hudson and Essex Counties. Collaborating with individuals or caregivers, we offer direct transportation for up to two hours per day.
Who is eligible for the program?
The program is available to adults of all ages who reside in the community and who suffer from health problems, problems related to chronic illnesses, or problems caused by the aging process. All clients must receive prior authorization from the New Jersey Department of Health to attend the program.
What is the cost to me?
If you have Medicaid, there will be absolutely no cost to you for any of our services, not even for our meals or transportation. If you are a private pay client, then a fee schedule will be given to you, but you pay only one amount no matter how many services you receive.
Who do I become a client?
By contacting our Admissions Department, you will speak to a staff member who will assist you with the admissions process and answer any questions you may have regarding your participation in the program. The admissions staff member will assist you in completing our admissions application and will arrange for a visit so you can see the program to ask further questions.
What is Prior Authorization?
The NJ Department of Health mandates that all applicants must meet specific medical criteria to participate in any Adult Day Care Program. Upon completing the program application, we will forward your medical information to the Department of Health for review. They will assess your application to determine medical eligibility for the program. Upon approval, we can enroll you as a client. Should you wish to transfer from another program to ours, we will notify your current center of your request and assist you in drafting a transfer request letter for submission to the State for approval.
What is the duration of the Admission Process?
Upon receiving your medical records and consulting with your physician, we conduct a Nursing evaluation. Typically, after 1-2 visits for the evaluation, we assess your eligibility and submit your application to the state for approval. During the waiting period for the State’s decision, you are welcome to participate in the program and access all our services, including transportation.
How is transportation provided?
We have our own vans and drivers. Our comfortable and safe vans ensure your safe transportation. Our drivers receive special training so that they can provide you with the best service. They will pick you up at your home and take you back home at the end of the day.
Can you provide transportation for medical appointments?
Certainly, just inform us in advance, and we’ll arrange transportation to and from your appointments.
What medical professionals are on staff?
Our program boasts professional nurses, a medical consultant, and personal care assistants. Additionally, we have on-site consulting medical and healthcare professionals who directly engage with our clients. These consultants include medical doctors, pharmacists, podiatrists, dietitians, physical therapists, and optometrists. We also collaborate closely with other medical professionals in the community to ensure you receive optimal healthcare.
Do I have to change my doctors?
No, you can keep all your current doctors, and we’ll maintain communication with them. They can provide us with orders for Nursing Care, and we’ll ensure seamless coordination with your primary doctor.
What If I need to go to the pharmacy to fill a prescription?
We collaborate with a local pharmacy, ensuring your prescriptions are filled and delivered directly to the program each day for your convenience.
What happens if I need help to get to the bathroom or to move about?
Our trained personal care assistants are equipped to aid you with your ADLs (Activities of Daily Living), including toileting, transferring, ambulating, grooming, and meal setup. Our entire staff is prepared to assist you as you navigate the program.
What happens if I need therapy?
If your personal physician or our in-house doctor determines that you could benefit from Physical, Occupational, or Speech Therapy, we will facilitate therapy sessions at a reputable establishment. Should you require a different form of therapy, we will collaborate with you to arrange sessions at a local therapy clinic.
